Quick Information and Checklist about Athletics
The 2011-2012 athletic season will begin on Thursday, September 8th. Football starts on August 29th. Get general information to help athletes register for a sport.
Sports Physical Form
Physical form filled out by parent/guardian and doctor. This needs to be completed every two years.

Student information and health history. This form is used by the school and your athlete’s coach for emergency issues.
