| Student information and health history. This form is used by the school and your athlete’s coach for emergency issues. |
| Concussion Information Sheet |
| The new Lystedt concussion law requires that all athletes and parents are informed of the symptoms of a concussion. LWSD now requires that all athletes and parents sign the Concussion Information Sheet that is attached to the above link. |
