Note: if you owe any outstanding fines, they need to be cleared before a transcript can be issued.

Complete "Request for Transcripts" form and drop it off in the Counseling Center
You may either pick up or have the Counseling Center mail your transcript. If we are mailing it, please be sure to include a stamped and addressed envelope along with your request for each transcript we are mailing on your behalf.
