To order your transcript

Note: if you owe any outstanding fines, they need to be cleared before a transcript can be issued.

Current Students: (download form)

  • Complete "Request for Transcripts" form and drop it off with an envelope to the Counseling Center. Pick up completed envelope within 5 business days.  You must mail envelopes yourself.

Previous Students:  (download form)

  • Fee is $5.00
  • Complete Transcript Request form and mail to registrar along with payment

  • You may  either pick up or have the Counseling Center mail your transcript.
