To be eligible to participate, all FIVE forms must be completed , the sports participation fee paid ($105.00), an ASB membership purchased ($25.00), and all fines paid. The necessary forms are available online on the "Athletics" page or in the Student Office. The required forms are 1) Sports Physical Examination/Clearance Form, 2) Athletics Emergency Information Form, 3) Middle School Athletic Policy, 4) Concussion Information Sheet, and 5) LWSD Athletic/Activities Drug-Alcohol & Tobacco Code. You must complete all of the above before a clearance card is issued.
