PTSA Membership Enrollment Form
Join PTSA! Print this form, fill it out, and turn it in along with your check to your child's teacher or the school office. To make sure the check doesn't get separated from your form, please use an envelope marked "Attn: PTSA".
PTSA Membership Discounts Brochure
Once you become a PTSA Member, you will receive a membership card that entitles you to discounts at local businesses!
PTSA Cash Reimbursement-Check Request Form
Use this form to request reimbursement for expenses approved by your Committee Chair or to request a check to pay expenses approved by your Committee Chair. Signature by the appropriate Committee Chair is required.
PTSA Deposit-Cash Tally Form
Use this form to tally your cash deposit. Two people are required to complete this form and sign it prior to turning it in to the Treasurer.
