Juanita Family Dance Ticket Reservation Form
Dance tickets for the night time dance on Thursday Evening, November 10th are $5 per person. (Children 4 and under are free as well as the Junior High School and High School Volunteers who are earning community service hours.)
 
Accept for jr. hi and high school volunteers, all students must be accompanied by a responsible, attending adult. Please NO drop offs!

    Download, print, fill out and send to school office with a check written to the Juanita Foundation for the correct amount.

In order to offset expenses to put on the Annual Family Dance, parents and students that wish to attend the dance need to purchase a ticket. 

Please download and print the form above and send in a check for the correct amount.

All Checks should be made out to The Juanita Foundation. 

Any profits from the dance go to the Music and PE Department which in turn spends earned dollars on programs and equipment for your child's Music and PE Program.
