has been read and understood by parents and the athlete

Middle Level Intramural Registration

Students are required to register for each intramural season at the school. Parents should contact the school directly for information regarding the registration dates.

Sport Specific Liability Forms - Each sport has a unique form that must be completed and signed before an athlete can begin practice - please contact your school's Athletic Secretary for this form.
