To be eligible to participate in any Lake Washington School District athletic program, a student must:

• maintain passing grades in all current subjects
• have at least a 2.0 GPA from the previous semester
• purchase an ASB card
• pay all fines and return all equipment/uniforms from previous sports

Once it has been determined that a student is eligible to participate in an athletic program, each of the following items must be completed to get Athletic Clearance:

• Physical Exam Card
• Verification of Insurance Coverage
• School Athletic Policy (request from the Athletic Secretary)
• WIAA Form
• Sport Specific Form (request from appropriate Athletic Secretary at your school)
• HIPPA Form (Disclosure of Protected Health Information)
• Pay the participation fee

These forms must be turned in together to your school's Athletic Office, along with a copy of your ASB card and participation fee receipt before the first practice.
