Immunizations and health forms

Required immunizations
There are specific vaccinations that are required by state law before your student can start school in the fall. Find out if your child is current on his/her vaccinations.

Required health forms
Some student health documents are required and must be completed and turned in to the school before your student can attend school in fall 2007. Additional required forms will be included in the first day packet that your student(s) will bring home from school.

Report serious health conditions
Parents/guardians of children with life-threatening health conditions must notify the school and complete the required paperwork before the student can start school in the fall.

Financial assistance for immunizations and sports physicals
Free or reduced cost health care resources and services, including medical exams, sports physicals and immunizations.
